Brittney Wilburn

Brittney works with the team and board to set the vision for the organization so that we can turn the vision into reality, ensuring community and client needs are met. She also strengthens collaboration with partners in the community working to improve the health and well-being across our 5 counties.
“I am thrilled to return to the safety net space and excited to support our team and communities in addressing thorny challenges with curiosity, fun, and design thinking.”
Brittney’s diverse background spans both clinical and operations, leadership, human centered design, issue advocacy, and grassroots organizing. She is guided by a strong sense of social justice, loves to raft rivers with her husband and three kids, read good books, and eat chocolate.

Jason Regan
Director of Operations & HR
Bio
Jason supports the internal operations of the organization and helps the team thrive through processes and structure. At his core, he has a servant’s heart and finds real joy in making others feel welcomed, seen, and valued. He brings positive energy, intentional care, and the occasional dry joke to every interaction.
Jason shares life with his incredible wife and their adventure-loving pup. He has climbed all of Colorado’s 14ers (and skied more than ten of them), completed several ultramarathons, including his first 100k in 2024, and never misses a chance to get outside. His travels have taken him to every continent except Africa and Antarctica.
He tries to live by this simple Ted Lasso wisdom: “Be curious, not judgmental.”

Sarah Meade

I work to support clinical and behavioral health providers in the community, in an effort to help individuals and families have access to the healthcare they need to thrive.
As a mom of 6 kids, I understand how important good healthcare can be, especially with so many individual needs. I have also seen how living in rural areas like ours can make this extra difficult. I hope my work with the Health Partnership helps ensure those resources aren’t so hard to find.
I have both a BSW and MSW in social work and have spent my professional career helping individuals of all walks of life, find resources they need to succeed in life.
Outside of work, I love hanging out with my husband and kids, working with the youth at church, listening to music and eating chocolate.

Ali Givnish
Community Impact Director
Bio
Ali has spent the past 8 years of her career working for Steamboat Springs Winter Sports Club, moving through the ranks to become the Youth Program Director (aka Circus Director) in 2022.
Ali is thrilled to bring her experience managing big groups of coaches, kids and parents to the deeply meaningful work of THP. Increasing access across the valley to health care and resources – in the big sense of the word – has long been a deeply held personal value of Ali’s, and the opportunity to be a part of that work is really exciting.
When she’s not tackling big complicated projects and organizations with millions of moving pieces, Ali loves adventures outside, most notably with her very fast husband Todd and annoyingly athletic son Theo.

Nele Cashmore
Recovery Program Manager
Bio
As the Recovery Program Manager for The Health Partnership, I am a Certified Peer and Family Specialist in the state of Colorado and provide peer support to anyone that may need assistance with substance use disorders and guide them to relevant resources within our community and beyond that may work for them in their recovery. With my own lived experience with alcoholism, I can provide empathy and encouragement to those who may need it. I also plan and host our RISE (Recovery, Insight, Support and Empowerment) Events in Craig which provide a space where people in recovery or those looking for support around substance use feel supported and encouraged. By bringing like-minded individuals together I hope to bring about change to our community that so desperately needs help with substance use and help break the stigma of addiction here in Craig by moving the needle one event, one conversation, one connection at a time. I also coordinate our efforts for The Yampa Valley Recovery Friendly Workplace Initiative. I coordinate efforts around planning and hosting fentanyl and naloxone trainings and community presentations locally, as well as plan and host the Colorado Rural Peer Conference and partake in state-wide meetings that address peer recovery support across the state of Colorado.
My most treasured accomplishments are my 4.5 years of sobriety and spirituality, becoming a mother to my two beautiful sons, marrying my husband and my most recent is acquiring this position with The Health Partnership and being of service to my community. Prior to these roles, I spent almost 20 years in the service industry which I love. I have learned skills from those years that I believe have shaped me into being the caring, giving, hard-working and driven person that I am today. In my spare time you can find me spending time with my family hiking, camping, fishing, 4x4ing, dirt-biking, skiing, practicing yoga, home projects, gardening and traveling.

Maryanna Younger
Care Coordination Program Manager
Bio
Maryanna works with our care team to offer an outstanding experience for clients as they go through care coordination services and are connected with local and statewide resources. “I grew up in Moffat County, using our community resources, and am now committed to helping others utilize these resources to better their lives,” she says. Maryanna brings a decade of care coordination experience. In her spare time, you will find her taking her old dog Lucy for car rides on dirt roads, watching cooking shows and experimenting in the kitchen with her family.

Kimberly Fox
Health Coverage Manager
Bio
Kimberly educates clients in Moffat, Rio Blanco and Routt county on available health insurance options and guides them through the enrollment process with ease. She has worked in a variety of healthcare settings as a Registered Dietitian and understands the frustration that comes with insurance. Kim is passionate about connecting clients with health insurance as well as essential overall health and treatment of disease needs. Outside of the office she enjoys reading, cooking, hiking, camping and spending as much time as she can with friends and family.

Emily Schweitzer

Emily works with organizations to create connections so that individuals have access to the resources they need.

Emily has a degree in Therapeutic Recreation and 10+ years of experience working with individuals with special needs. This has helped her connect with the community and recognize gaps in resources. This has developed a passion for filling those gaps!
In her spare time, you can find her with her pup, Maci, skiing, mountain biking, puzzling, boating on Stagecoach Reservoir or simply spending time with friends.

Elizabeth Heckmann
Grants Manager
Bio
I help secure diverse grant funding for THP and manage grants through their entire lifecycle. I was drawn to work with The Health Partnership after working with a local community mental health organization for two years allowed me to see the amazing work done by the entire THP team.
I earned my BS in Equine Science from Colorado State University and my MFA in Creative Nonfiction from Goddard College. I also worked as a Certified Nurse Assistant for seven years, and I earned my Colorado Family and Peer Specialist certification in 2022. Outside of work, I love spending as much time as I can riding horses, birding, playing my flute, relaxing with my husband and our cat, and practicing Wing Chun.

Dana Antonio
HOPE Initiative Project Manager
Bio
I work with clients in Routt County and beyond who specifically need support with behavioral health and mental well-being resources. I help connect these individuals with mental health professionals and other services in our area. It is my hope that the work I do at The Health Partnership will contribute to the greater goal of improving the mental well- being of our community as a whole.
My background is widely varied but has always placed a strong emphasis on helping others, whether as a Human Services case worker or Wilderness EMT instructor. I have a bachelors from the University of Colorado in Physiology, I am certified as an Emergency Medical Technician (EMT), and I am a Nutritional Therapy Practitioner (NTP).
In my free time you can probably catch me outside, usually mountain biking on local trails in summer or backcountry skiing and adventuring in winter. I also love to read, try new recipes, and plan road trips that involve soaking in hot springs and camping off the beaten
path with my husband and dog.

Maria Jose Demara

As the Social Health Information Exchange (SHIE) Project Manager, I oversee the development and implementation of the SHIE initiative, working closely with partners to advance connected care and community collaboration. In addition to managing the project, I am actively involved in the hands-on configuration and development of Channels360, ensuring the platform meets the needs of organizations and the individuals they serve. I am passionate about using technology to improve data sharing, streamline coordination, and support better outcomes across health and social care systems.
In my spare time I enjoy hiking our beautiful trails, mountain biking, drawing and coloring, playing basketball and watching Korean dramas.

Marissa Jaime, Routt County
Care Coordinator
Bio
Marissa works with members to develop their care plans, provide education and connect them to resources. She credits her previous work with Department of Human Services for preparing her to help people from all walks of life be self-sufficient, elevate their social standings, and lead them to live better and healthier lives. Marissa graduated from St. Paul College in Manila, Philippines, with a bachelor’s degree in psychology and business administration. In her spare time, she loves gardening and home decorating.

Shannon Doin

I work with clients in Routt County to connect them to health and wellness resources to reduce barriers to care and achieve care plan goals. I have found a deep investment in the wellbeing of the community through work involving vulnerable populations
here in Routt County. I have my Bachelor’s Degree from St. Lawrence University in Biology and Government. In my spare time, I love to be outside skiing or running, and spending time with friends.

Michelle Wilson

I work with clients in Moffat County to connect them with resources they may not be aware of or not be able to obtain on their own. I am fourth generation Routt County and am very proud of my deep roots here in the valley I’ve worked as a care coordinator in similar fields over the last seven years. In my free time, I’m usually taking care of my plants, reading books, or enjoying the outdoors.

Camille Kline
Behavioral Health Clinical Care Coordinator

At The Health Partnership, as a Behavioral Health Care Coordinator, I work with clients who are struggling with mental health and require assistance to connect to mental health professionals and local resources which can provide assistance for their health and well-being.
Although I am not originally from this area, I have grown to love the Yampa Valley and I want to help the people in our community get the services they need. I have a Bachelor’s of Science from BYU-Idaho, with certificates in Human Services and Relations, and Marriage and Family Functioning. I also have a background as a caseworker in Child Welfare. When I am not working hard to help the community, I enjoy baking and taking road trips with my amazing husband.

I work with Spanish-speaking individuals or families to support them in getting connected to the resources they need. I also work collaboratively with a variety of community partners to ensure equitable access to resources in the community.
I am so excited to be part of an organization, like The Health Partnership, where I can give back to the community by promoting the resources that we have in the area that will help families to find that support they need.
I love life, family, friends and great conversations. In my spare time, you will find me enjoying the outdoors, skiing, camping or just spending time with friends.

Brook Mower
Peer Recovery Specialist
Bio
My job is to help individuals in recovery connect with community resources and support them in their recovery journey. I provide a non-judgmental space for individuals to express their feelings and experiences. I also host sober events for the community that I have spent my entire life in. I’m super excited to be able to help others in recovery, as I am in recovery. Overcoming obstacles is very scary, and it’s nice to know you’re not alone. My life goal is to mix peer support and paralegal work to help people who have been in the position I was once in.
When I’m not at work, you can find me working on the ranch, spending time with the animals, doing crafty things, and hanging out with my family.

I am a Care Coordinator at the Health Partnership. I help connect individuals to resources within our community.
I have chosen to take the role of Care Coordinator because through my own life experiences I have learned firsthand the significance of having access to resources. I have a passion for helping and serving others.
When I am not working you will see me out enjoying my free time with my children. I love all the events and festivals we get to enjoy in our small town. I am blessed to be in a community with plenty of access to the outdoors!

Alison Rosello

I am a dedicated Peer Recovery Specialist with a strong passion for supporting individuals on their journey to overcome addiction. Drawing from both personal experience and professional training, I strive to create a safe, empathetic space where people feel seen, heard, and empowered. As a bilingual professional, I have the opportunity to connect with and assist both English- and Spanish-speaking individuals and families, helping them access the resources and guidance they need during challenging times.
Outside of work, I enjoy spending my free time reading, traveling to new places, and going on hikes with my dogs. These activities help me recharge and stay grounded, allowing me to bring my best self to the people and communities I serve.

I’m Claudia, and I’m super excited to be THP’s Health Coverage Coordinator! Prior to joining THP in November of 2025, I worked closely with healthcare teams, supporting a wide variety of patients from all different ages and backgrounds. I truly love connecting with people and finding ways to make their healthcare experience a little easier.
I take pride in building genuine connections with those I work with and always try to bring positivity and energy to the workday. I think it makes the day more enjoyable for everyone!
Outside of work, I enjoy cooking for my family, reading, hiking, and I’m a total fanatic about exploring off-roading trails!

As a Peer Recovery Specialist with The Health Partnership in Steamboat Springs, Colorado, I use my lived experience with recovery to provide compassionate non-judgmental support to individuals navigating substance use and mental health challenges.
I am dedicated to walk alongside others as they build healthier, more stable lives, offering hope, encouragement, and practical tools for long-term recovery. My role focuses on meeting people where they are, helping them identify personal goals, access community resources, and develop skills that support independence and well-being. I believe recovery is possible for everyone and that connection, dignity, and empowerment are essential parts of the healing process in recovery. Through advocacy, mentorship, and peer support, I strive to create a safe space where individuals feel heard, respected, and motivated to move forward.It is an honor to support members of our community as they rediscover their strengths and build meaningful lives in recovery!
Outside of work, I love staying active at the gym with my husband, exploring the outdoors, and walking my dog, Mason!

Emily supports the executive team by helping build strong internal systems and
keeping day-to-day operations running smoothly. She focuses on the small details
so the team can stay efficient and focused on achieving long-term goals that create
a more accessible, easier-to-navigate health and wellbeing experience for the
community.
After working for a similar organization in her hometown in California, Emily
developed a passion for advocating for a more equitable and accessible care
system, driving meaningful change to destigmatize mental health and better
educate people on the power of asking for support. She is excited to apply that
experience in a new environment and deepen her understanding of the challenges
across Northwest Colorado. Emily is eager to invest her time, energy, and skills into
supporting and strengthening the community she now calls home.
In her free time, you’ll likely find Emily baking treats to share with friends & family,
out on the slopes, exploring new hiking trails, strolling the farmers market, or
soaking up some sunshine by a lake with a good book in hand.

Hi, my name is Monica Schultheis. I have worked with children, families, and communities to help reduce barriers and connect people to the support they need. I’m currently earning my B.S. in Psychology in Human Services and plan to continue into graduate studies in Social Work.
My experience in education and family support has shaped my passion for empathy, advocacy, and coordination. This work feels very meaningful to me because of my belief that people make the most progress when they feel heard and supported.
Outside of work, I enjoy volunteering, creative art projects, being outside, gardening, reading, and cooking.

Ashley Ellis

I work with clients primarily in Routt, Moffat, and Rio Blanco Counties to connect them with resources, support them, and help them navigate whatever circumstances they are in. I’m so excited to work for The Health Partnership – I’ve had the opportunity to collaborate with several of the staff and participate in a few of the great events (At the Table, COPE, and the Wellness Conference). I’ve admired the work being done and am excited to join this momentous team. I have a B.S. in Psychology, am a Certified Master Grief Coach, and bring lived experience, and training in suicide awareness/prevention and peer support. I’m trained to facilitate Talk Saves Lives, QPR, and COMET. In my spare time, you’ll find me at home with my husband, three dogs, a few chickens, and various bonus children watching some random “leaving soon” movie on Netflix, trying to garden, or walking the dogs.

I have a bachelor’s degree from Bellevue University in Behavioral Science and over 5 years of experience working in health care and substance abuse. In my free time I enjoy spending time with my family outdoors and going to the movies.

Lucy Winslow
Regional Health Connector/CO SHIE Outreach Coordinator

As a Regional Health Connector, I work with community partners in Routt, Moffat, Rio
Blanco. Grand, and Jackson counties to provide access to affordable care through the
distribution of resources. As the CO SHIE Outreach Connector, I guide partners through
the CO SHIE program, from education, installation, and use.
My professional career has always surrounded advocacy specific to outdoor access,
and I am so excited to take the next step into the important world of public health.
Without our health, we cannot do the things that bring us joy – and everyone deserves
that! I’ve always been the black sheep of the family by not being in healthcare and
pursuing steep lines, so it was about time I made my arc.
I have my Bachelor of Science in Psychology with an interest in Social Dynamics, plus
minors in Business Administration and Environmental Studies. I am Class of 2025
Leadership Steamboat.
When I’m not busy connecting communities, you can find me in the trees on my bike or
my backcountry skis with my goofy pup, Grisman. I’ve also been taking my side quests
for knitting and ceramics pretty seriously and am an avid live music lover.

Brittney Bartholomew

Hello, my name is Brittney Bartholomew. I work with clients in Rio Blanco County, Colorado. I work with the Health Partnership because I believe in their mission to help people through all walks of life. I am especially passionate about the work they do with the recovery community, because I have faced addiction and witnessed most of my family struggle with it as well.
I have seen the benefits of helping others get sober and feel it’s important for people in active addiction to know that hope is not lost, and it’s never too late to recover. I can assist you with your recovery journey by being a support person, checking in with you regularly, and helping you with information on resources available to assist you. I am currently attending college, working on a degree in criminal justice with a focus on psychology.
In my spare time, I like to spend time with my family and make the most of every day together. I am a true crime junkie and really enjoy documentaries.

Sofia Rodriguez

I’m passionate about making a positive difference in my community and being part of a team that supports the health and well-being of others. Through my work with the Health Partnership, I’m excited to connect with people, build meaningful relationships, and contribute to creating a healthier, stronger community.
Outside of work, my favorite role is being a mom. I enjoy spending quality time with my kids and making memories together. Whether we’re enjoying outdoor activities, going on family outings, exploring new places, or simply spending time together at home, my family keeps me motivated and grounded. I believe that creating healthy, happy moments with my children is one of the most rewarding parts of my life.
I’m excited to bring my positive attitude, dedication, and love for helping others to the Health Partnership while continuing to grow both personally and professionally.
