The Health Partnership ensures all residents in the Yampa Valley have access to primary care & behavioral health services, recovery support services, housing, transportation & nutrition. We connect the dots between people, providers, and community resources, working alongside local partners to fill gaps, reduce barriers, and prevent duplicative efforts. By strengthening the network of support across our communities, we help ensure that no one falls through the cracks and that every person has the opportunity to live a healthier, more connected life. All of our services are completely FREE with no barriers to access.

Your donation helps us:

• Connect individuals and families to healthcare, behavioral health, housing, food, transportation, and other essential services
• Support recovery, care coordination, health coverage, and judicial support programs across the Yampa Valley
• Strengthen partnerships between local organizations to create a more seamless system of care
• Identify community needs and lead collaborative solutions that improve health and well-being for everyone
• Advocate for underserved populations and ensure every person has the opportunity to thrive

• $25 → helps provide transportation support for a client
• $50 → helps cover supplies for a RISE (Recovery, Insight, Support, and Empowerment) event for our sober & sober-curious community
• $100 → helps support professional training for a staff member
• $250 → helps provide a conference scholarship
• $500 → helps support access to therapy or another direct service
