Opening Doors
The Health Partnership serves as the hub, bringing people, organizations, and resources together to create healthier, more equitable communities. We work to identify and address the economic, cultural, and health-related barriers, as well as the stigma that can prevent people from seeking support and accessing the care and resources they need.
In partnership with public health organizations and community leaders, we identify gaps in health and well-being and bring people together to develop collaborative solutions. By strengthening connections across systems and opening doors to resources, we help build communities where everyone has the opportunity to thrive.
Our Top Priorities
Provide high-quality community-based care coordination.
Provide recovery support through a highly-trained team and organize sober social events to help create community and break through stigmas related to substance use disorders.
Provide leadership and facilitate collaboration to assess, educate, and advocate across the region on factors that impact community health and address inequities.
