The Health Partnership serves as the hub to ensure that systems are working efficiently and effectively to help people overcome economic, cultural and health related barriers, including any perceived stigma, and reduce health inequities by opening doors to resources in the communities we support.
Working closely with our public health partners, we identify gaps in health and well-being within our communities and facilitate collaborative solutions to close those gaps.
