The Health Partnership serves as the hub to ensure that systems are working efficiently and effectively to help people overcome economic, cultural and health related barriers, including any perceived stigma, and reduce health inequities by opening doors to resources in the communities we support.
Working closely with our public health partners, we identify gaps in health and well-being within our communities and facilitate collaborative solutions to close those gaps.
Our Top Priorities
Provide high-quality community-based care coordination.
Lead collaborative initiatives to address social factors that negatively impact health across the region.
Provide leadership and facilitate collaboration to assess, educate and advocate across the region on factors that impact community health and address inequities.
We foster equitable access to health and well-being resources so our community can thrive.
To help people live longer, healthier lives.
