Integrated Community Care Coordinator | Moffat County

Help community members navigate healthcare systems, access services, and create personalized care plans that support their overall well-being.

Peer Recovery Specialist | Rio Blanco County

Support individuals and families impacted by substance use disorder through recovery coaching, peer support, community engagement, and RISE event programming.

Care Coordination Program Manager | Steamboat Springs or Craig (Hybrid)

Lead and strengthen our Care Coordination Program while supporting staff, building partnerships, and advancing health equity throughout Northwest Colorado.

Health Coverage Coordinator | Part-Time | Steamboat Springs or Craig (Hybrid)

Help community members understand health insurance options, access financial assistance programs, and connect with coverage that meets their needs.
